Ipinapakita ng aming listahan ng mga gamot (pormularyo) ang mga gamot sa Part D na saklaw namin. Sa pangkalahatan, saklaw namin ang inyong gamot kung ito ay medikal na kinakailangan. Saklaw ang mga gamot na nasa aming listahan ng mga gamot kapag ginamit ninyo ang mga parmasyang nasa network namin o ang aming serbisyo sa mail order para sa mga maintenance na gamot. Ang mga maintenance na gamot ay ang mga gamot na iniinom ninyo para sa chronic o pangmatagalang kondisyon. Ang ilang gamot na saklaw namin ay may mga limitasyon o iba pang patakaran.
Ang Pharmacy and Therapeutics Committee, ang independent naming team ng mga eksperto sa pangangalagang pangkalusugan, ang sumusuri at nag-aapruba sa aming listahan ng mga gamot. Hindi lahat ng gamot ay isinasama namin. May ilang gamot na posibleng hindi saklaw o kaya ay nakabukod. Paano Ako Maghahanap ng Gamot sa Listahan ng Mga Gamot (Pormularyo)?
Puwede kayong humanap ng gamot gamit ang aming Tool sa Paghahanap ng Gamot o sa pamamagitan ng pagbubukas ng dokumento ng Listahan ng Gamot (Pormularyo). Nagbibigay ang bawat opsyon ng kumpletong listahan ng mga sinasaklaw na gamot at anumang paghihigpit o limitasyon. Ipinapakita rin sa inyo ng tool sa paghahanap ang mga alternatibo sa sinasaklaw na gamot.
The PDF document lists drugs by medical condition and alphabetically within the index. To search for your drug in the PDF, hold down the “Control” (Ctrl) and “F” keys. When the search box appears, type the name of your drug. Press the “Enter” key. You also have the option to print the drug list as a PDF document. The drug list is updated monthly.

Abiso ng Pagbabago sa Listahan ng Mga Gamot
Drugs may be added or removed from our list of drugs during the year. Generally, we will tell you before we make one of the following changes to the list of drugs:
- Mag-alis ng gamot sa listahan.
- Mga kinakailangan sa pagbabago ng gamot.
- Maglipat ng gamot sa mas mataas na tier ng bahaginan sa gastos.
Kung sasabihin ng Food and Drug Administration (FDA) o ng gumawa ng gamot na hindi ligtas ang gamot, aalisin ito agad sa aming listahan ng mga gamot. Bukod pa rito, kung may papasok na bagong generic na gamot sa merkado, baka alisin namin ang branded na gamot.

Paunang Pahintulot, Step Therapy, at Mga Limitasyon ng Dami
- Paunang Awtorisasyon: Kailangan namin na humingi muna kayo ng pahintulot sa amin bago kami pumayag na saklawin ang ilang partikular na gamot. Tinatawag namin ito na paunang pahintulot. Kung hindi kayo aaprubahan, puwede kayong singilin para sa gamot. Ang mga gamot na nangangailangan ng paunang pahintulot ay may “PA o PA-NS” sa Listahan ng Mga Gamot (pormularyo).
- Step Therapy: May ilang pagkakaktaon kung saan hihingiin muna namin sa inyo na sumubok ng ilang partikular na gamot para gamutin ang inyong medikal na kondisyon bago namin saklawin ang iba pang gamot para sa naturang kondisyon. Ang pangangailangang ito na sumubok muna ng ibang gamot ay tinatawag na step therapy. May nakalagay na “ST” sa mga gamot na nangangailangan ng step therapy sa Listahan ng Mga Gamot (pormularyo).
- Mga Limitasyon ng Dami: Para sa ilang partikular na gamot, nililimitahan namin ang dami ng gamot na sasaklawin namin. Halimbawa, isang tableta kada araw. Posibleng dagdag ito sa karaniwang isang buwan o tatlong buwang supply. Ang mga gamot na nangangailangan ng mga limitasyon ng dami ay may nakalagay na “QL” sa Listahan ng Mga Gamot (pormularyo).

Mga Pagpapasya sa Pagsaklaw sa Gamot
You can ask us to make an exception to our coverage rules for your drug(s). To learn about the types of exceptions, refer to your Evidence of Coverage. When asking for an exception, include a statement from your doctor that supports your request, plus a completed Coverage Determination form.
Generally, we must decide within 72 hours of getting your doctor’s supporting statement. You or your doctor can request a fast (expedited) exception if your health may be harmed by waiting. If we approve your expedited request, we must give you a decision within 24 hours after we get your doctor’s supporting statement.

Mga Generic na Gamot
We cover both brand name drugs and generic drugs. Generic drugs have the same active ingredient formula as a brand name drug. Generic drugs are FDA-approved, and are as safe and effective as brand name drugs. They have the same active ingredients, indications, dosages, safety, and strengths as the brand name drugs and generally cost less. Ask your doctor if any of your drugs are available as a generic, and if a generic version will work for you.
