Find Your Plan
To get started, use the Find Your Plan tool. Enter your plan type, estado, and county to search for your plan. Then, select your plan from the list of available options.
How to Locate Plan Details and Documents from the Plan Overview Page
- Plan Documents: locate your Annual Notice of Change, Summary of Benefits, and Evidence of Coverage documents in the “Plan Specific Documents” section of the page. You can also locate the Appointment of Representative and Pharmacy Direct Member Reimbursement forms in the "Related Materials" section of the page.
- Drug List (Formulary) and Prescription Drug Claim Form: You can locate your plan’s Drug List (Formulary) and Prescription Drug Claim Form by selecting “Pharmacy” from the right sidebar. From the “Pharmacy” drop-down menu, select the “Drug List (Formulary) and Other Documents” page.
- Pharmacy Resources: Locate additional Pharmacy information such as the Medication Guide, Medicare Extra Help, Specialty Pharmacy, Mail order Service, and Transition Policy by selecting “Pharmacy” from the right sidebar. From the “Pharmacy” drop-down menu, select the additional Pharmacy information page that you’re interested in reviewing.

- Member ID Card – view print, or order a replacement card through the secure member portal. In a hurry? View or print a temporary ID card (PDF) now!
- Find a Provider/Pharmacy – find a provider or pharmacy using our search tool.
- Pharmacy Directories – access Pharmacy Directory documents.
- Real Time Benefit Tool – to access the Real Time Benefit tool, login to the secure member portal. If you don’t have an account, select “Create New Account” and follow the prompts.
- Medicare Disenrollment – use our online disenrollment form to request a disenrollment from the plan.
