We are now offering a support program to eligible Chronic Kidney Disease (CKD) and End Stage Kidney Disease (ESKD) members that is designed to help improve their quality measures and clinical outcomes, resulting in more healthy days for our shared patients.
This program provides our eligible members, with or at risk of CKD/ESKD, with a personal support team of health professionals to help them manage their kidney disease and actively follow your treatment plan. We partnered with a leading value-based kidney care organization, Somatus, for delivery of this program. The goal is to keep patients out of the hospital, improve their quality of life, and slow or stop disease progression.
TThe Somatus healthy living program is already part of these patients’ insurance package and is available to them at no extra cost.To find out which patients in your practice are eligible for this program, please email provider@somatus.com.
The Somatus team supports your patients through:
- One-on-one care to help manage their kidney disease and comorbidities and address social determinants of health
- Personal health coaching that is based on their condition, treatment options, and diet
- Assistance to transition safely from hospital to home
- Guidance exploring transplant options, if appropriate
