6 Degrees Health offers convenient options to submit records

In March 2024, Meridian and its family of plans transitioned from Optum as our clean claim vendor to 6 Degrees Health. Through 6 Degrees Health, providers can submit records via mail, fax, or email.
6 Degrees Health oversees clean claim reviews for:
- Meridian Medicaid Plan
- YouthCare HealthChoice Illinois
- Meridian Medicare-Medicaid Plan (MMP)
- Wellcare Medicare

- What are EX CODES?
Certain EX CODES means a clean claim review was completed. If EX CODES n0, n9, m0, m3, m4, or m5 are on the EOP, 6 Degrees Health conducted a claim review.
